ヘバーデン結節　　ブシャール結節　テーピング

Heberden's nodes

へバーデン結節は人差し指から小指にかけて第一関節が変形、ブシャールは人差し指から小指にかけて第二関節が変形します。40歳代くらいから発症し女性に多くみられます。女性ホルモンが関与していると考えられています。関節の変形はあるので、リウマチかなと心配なされる方もいますが、リウマチとは異なります。リウマチは血液検査でリウマチ因子を調べて特定しますが、へバーデン結節では、リウマチ因子は上がりません。指の変形は、一旦落ち着くと、痛みも治まりますが、また、別の指の関節が変形し痛み出していき、繰り返していくことで、人差し指から小指にかけて徐々に変形していきます。治療に関してはテーピングが推奨されています。テーピングをすると、関節が固定されるので痛みを感じにくくなります。また、テーピングを巻いていると気持ちも落ち着くようです。 寝る前に貼って寝ると、朝は 痛みが軽減されるたという患者さんもいました。注意事項としては、指の周りにテーピングを巻くので、巻いた後、強く巻きすぎて、血流が悪くなっていないか不快な感じがないか爪の色を確認し、むらさき色になっていないかなどの血行障害の確認をします。

Heberden's nodes deform the first joint from the index finger to the little finger, while Bouchard's nodes deform the second joint from the index finger to the little finger. Female hormones are thought to be involved. Some people worry that they have rheumatism because of the deformed joints, but this is not the same as rheumatism. Rheumatism is identified by blood tests for rheumatoid factor, but rheumatoid factor is not elevated in Heberden's nodules. Once the finger deformity settles down, the pain subsides, but then the joints of other fingers become deformed and painful again, and the deformity gradually progresses from the index finger to the little finger through repetition. Taping is recommended for treatment. Taping helps to immobilize the joint so that it is less painful. Taping also seems to calm the patient's mind. Some patients say that if they put the taping on before going to bed, they feel less pain in the morning. As a precaution, since the taping is wrapped around the finger, after wrapping it, check the color of the nail to see if it is wrapped too tightly and if there is any discomfort, or if the blood flow is impaired, and if the color of the nail is purple or otherwise impaired.
